Request for Change 更改保單資料

I/We request that the policy be changed according to the above particulars. I/We understand and agree that this request will form part of the policy if accepted by Insurer. I/We further understand that this request is NOT valid until it is finally confirmed and accepted by Insurer by Endorsement or letter/email.
本人/吾等在此要求保單按以上資料更改, 並明白及同意若更改申請獲保險公司接納，將會構成保單契約之一部分。本人/吾等明白此申請最終需經保險公司批註或以信 件/電郵確認方為有效。
